Reminder:  Please schedule your child’s appointment with the doctor as soon as possible and definitely before leaving for summer vacation.  August is a month that is packed with parents requesting appointments and most physicians will not be able to accommodate a last minute request prior to the start of school. Don’t forget to make copies for your records. Once submitted, you will not be able to retrieve the forms.
